I herby waive any claim of liability against Next Level Hitting Instruction and Payless Batting Cages, their officers, and all staff for any injury arising from any cause whatsoever, which occur by reason of my none presence, and further authorize any and all medical and hospital care and treatment, including major surgery, deemed necessary by the personnel of any medical facility, for the health and well being of my child(ren) aforesaid.
